Essential Criteria (must have):

Experience working with young people in supported housing, community support, or a similar setting:

Minimum 1 years’ experience, OR

A relevant degree (e.g. Social Work, Youth & Community Work) plus 6 months’ experience

Experience of risk assessment and safety planning
Experience of needs assessment and support planning
Ability to work independently, use initiative and manage a varied workload
Strong communication and interpersonal skills
Resilience and compassion when working in a challenging environment

Desirable:

You will build trusted relationships, coordinate multi‑agency support through the Complex Lives MDT, and lead on developing, implementing and reviewing personalised support and risk management plans. The job includes maintaining accurate records, contributing to audits and quality improvement, working collaboratively with partner agencies, and ensuring services meet regulatory and organisational standards.
You’ll also participate in a 24‑hour shift rota (including nights and weekends), support health and safety procedures, promote equality and client choice, and help drive continuous improvement across the service.

The Person:

Minimum 3 years’ experience working with socially excluded or vulnerable individuals.
Proven ability to build strong relationships with clients, colleagues, and partner agencies.
Experience delivering person‑centred, strengths‑based support, including assessing needs, risks and goals.
Ability to plan, prioritise and manage your own workload effectively.
Confident using Microsoft Office (Word, Excel, PowerPoint, Outlook)
