  • Liaise with hospitals, community services, voluntary agencies, and relevant stakeholders to ensure coordinated and integrated care.
  • Participate in integrated care pathways and frailty initiatives, including joint assessments where required.
  • Contribute to discharge planning, advocacy, and community reintegration.
  • Provide clinical leadership, supervision, and support to junior staff, therapy assistants, and students.

Requirements

  • CORU registration as an Occupational Therapist (essential).
  • Recognised CORU-approved qualification in Occupational Therapy.
  • Preferably have some adult primary care, adult medical, adult disability, seating, complex cases, neurological or palliative care experience.
  • Access to a car (almost all work is taking place in service users homes in / around the Ballyfermot / Palmerston / Inchicore / Cherry Orchard area).
