Responsibilities:

  • Manage a varied, rehabilitation-focused caseload including patients with ABI, stroke, and post-acute care needs.
  • Conduct comprehensive functional and falls risk assessments for residents.
  • Develop and implement individualized rehabilitation treatment plans to improve quality of life.
  • Lead specialised group exercise therapy sessions tailored to varying physical and cognitive abilities.
  • Collaborate with Occupational Therapists and nursing staff to provide a holistic care approach.
  • Deliver evidence-based physiotherapy interventions to optimise mobility, function, and independence
  • Conduct comprehensive physical, functional, and mobility assessments
  • Develop and implement individualised treatment and discharge plans
  • Support patients with rehabilitation, reablement, and safe transitions to home or community care
  • Work closely with patients, families, carers, and multidisciplinary teams
  • Participate in MDT meetings, service development, and clinical audits
  • Provide guidance, mentorship, and supervision to junior staff where required

Requirements:

  • CORU-registered Physiotherapist
  • Minimum 3+ years post-qualification experience, ideally in rehabilitation, ABI, stroke, or older persons care
  • Strong experience in complex case management and discharge planning
  • Excellent clinical reasoning, communication, and teamwork skills
  • Experience working within multidisciplinary teams
  • Previous leadership or mentoring experience is desirable
  • Full clean Irish driving licence and access to a car
  • Prior experience in a nursing home, residential, or step-down setting is a distinct advantage.
